Regular Visitor Posts: 413 05/02/08 10:46 AM	Remove this ad Well, I had to go and mention the 2 car accidents, didn't I... it was in self defense though, I was explaining that I was in two tramatic events in a year with no anxiety so why the hell would it start now? But I guess tangling with a semi in a snow storm then being taken out by a pick-up truck in a parking lot within a year (neither my fault, thank you) doesn't do much for one's psychological evaluation. It seems that even rocks like myself get worn away by the river of stress and the accidents just may contribute to my current mental state. I'm upping the ole Citalopram, but I don't mind. I'll see what happens and if it doesn't help, I'll just taper off again. I'm on a schedule to get up to 50mg, I guess I have some anxiety issues to deal with. I asked and was told I have not been diagnosed with GAD, something more like PTSD, but not PTSD... He also mentioned temporal lobe seizures in an off hand manner but then decided that my neurologist would be the best to handle the DP etc... problems which I had started to assume were psychological but I guess aren't... Talk about confusing. The Semi changed lanes into me, quick thinking and I got mostly out of his way, but he did a little damage to my rear bumper which I kindly offered to let the driver pay for out of his own pocket rather than lose his job and possibly his license. The pick-up didn't stop at a stop point in a parking lot and totalled my car. I didn't see him coming due to the "plethora" of tinted windowed mini-vans. (ok the one mini van)

I don't really get where the pshychiatrist was going, but I guess in certain people things can take years to digest... like a 16oz steak. I admit I was acting weirdly after the accident, all I could think of was, yay, now I get to go car shopping. Well.. it WAS spring afterall!!

Can you check whether you had / or have / any of the PTSD symptoms mentioned in DSM-IV? http://www.ncptsd.va.gov/ncmain/ncdocs/fact_shts/fs_dsm_iv_tr.html Does your psychologist (psychologist or psychiatrist?) assume that your current symptoms (with the perception symptoms) are due to PTSD with delayed onset? Regular Visitor Posts: 413 05/04/08 8:34 AM	Hi KSomething I can't recall the exact dates - I'll see if I can find the information. ca. January 2007 - tangled with tractor trailor in snow storm ca. May 2007 - pickup truck totalled car The psychiatrist indicated that I don't have PTSD, but rather something in the same catagory or family is causing anxiety problems, ie. current stress and past trauma are building up and causing problems. He believes that I have two conditions, a psychological and a physical condition. The physical condition would be the migraine condition, the psychological would be parallel and aggrivated/triggered by the migraine and past accidents. From what he said, I gather he doesn't believe the vision problems, the weakness, or the derealization are psychological. Just the wicked mood swings, feelings of extreme frustration, irritability, etc... he says things have built up and finally became too much for me to digest. I don't really know, but I'll try upping the citalopram and see what happens. I do not meet any of critereon C or F, but other than that I meet the rest. We did discuss Criterion B: intrusive recollection at length, it was much worse closer to the accidents, but I do still occasionally have the occasional flashback. Cheers, Jim

It was explained as similar to PTSD, but not quite. What do you think of temporal lobe seizures for derealization? I don't recall having smells or tastes, but there is some DEJA VOUS and I do stare and it is really hard to explain what the experience is like or interact ith the world. The majority of the attacks come during or immediately after eating. I think this is why he made comment (then seemed to regret making it). Just how much overlap is there between epileptic seizures and migraine aura attacks? I understand some studies show there is significant comorbidity between epilepsy and migraine, but not between migraine and epilepsy (it seems to be a one way connection, people with epilepsy have been shown in studies to have a high rate of migraine as well, though perhaps over-diagnosed or mis-diagnosed, while migraine sufferers are only slightly more likely to be epilepsy sufferes than the general public) But when I read up on partial temporal lobe seizures, I really start to think, wow, is there a grey fuzzy area between the two diseases and am I in it?

Registered User Posts: 5010 05/04/08 9:25 AM	One can make a diagnosis of subsyndromal = partial PTSD (see http://www.migraine-aura.org/content/e27891/e27265/e42285/e42290/e55289/e61598/index_en.html ) Both the low severity of your PTSD symptoms and the long latency between the trauma and the onset of the (2nd) persistent aura episode suggest that the trauma didn't play a role as a trigger of persistent aura here. The subsyndromal PTSD symptoms might have contributed to the depressive episode, however. "It was explained as similar to PTSD, but not quite. " = subsyndromal PTSD "I was not diagnosed with either GAD or PTSD. It seems to be a trauma and stress induced depressive/anxiety thing, if that makes sense?" Depressive episode seems to fit, and one can add, on a descriptive level, that the subsydromal PTSD symptoms add to the depression symptoms SSRI treatment is therapeutic consequence anyway with all of these differential diagnostic considerations

Regular Visitor Posts: 351 05/04/08 10:07 AM	I have many PTSD symptoms...but i've never bothered telling any doctors...mainly because of how bad my doctors are! Car accident was in 2003, started with being shot at (wrong place wrong time)...2 bullets hit side of the car, 1 skimmed side of my head (left permanent scarring) and half of one (after it hit the car?) managed to hit me in the face and jam itself fracturing my bottom jaw...followed by car chase...then me crashing (clocked at 112MPH at time of crash)...then a 4 hour ordeal with the police (interview, cell, finger prints, drug check) before i was allowed home...and didnt get to hospital till the next morning (police wouldnt let me go...ambulance men were stood right there when police said "he isnt going anywhere")...left a huge mark on me...i had nightmares for months after...on top of that i got taken to court for twok (was my dads car, i wasnt actually driving it, just listening to music as no stereo in the house due to house fire)....all this happened on my own drive way (private land)...was facing upto 6 months inside...very stressful upsetting and depressing time for me...luckily got found not guilty (whole case took a good 8 months)....after all that i rarely left the hosue (too scared those guys might come back and finish me off)...and lived with a baseball bat at both doors...and one by my bed...i was on edge for a really long time...life got alot worse after that (moved) where multiple things happened that effected me considerably....so really the last 4 years of my life have been nothing but hell....i believe my VS is my bodys way of saying "i've had enough"

Regular Visitor Posts: 413 05/04/08 12:43 PM	I don't have any pictures of the first accident. I didn't have a cellphone with a camera at the time It was just damage to the rear bumper skin. Here is a Gif of the accident. The big black blob is the semi, the little black blob is my car. The long line is a concrete barrier. It was snowing badly, the road was bad, traction wasn't good, but I managed to almost get out of the way, somehow I didn't spin out, I just kept on the power and luckely I grew up doing stupid car tricks in the winter in mall parking lots, so I recovered. I'm sure a lot of people would have ended up crunched into the barrier or under the front of the truck. Here is the 490 width.

Regular Visitor Posts: 413 05/04/08 8:05 PM	Getting back to the temporal lobe seizures, the Aura does resemble some of the transitory persisting migraine aura symptoms I have. Vertigo Auditory hallucinations DP / DR Nausia It does resemble the Derealization attacks that I have: Occurring more often after a meal, but can happen at any time: Aura: Derealization Fear/Anxiety déjà vu or jamais vu Seizure: Breif Motionless Stare - little or no control over body Postictal: Confusion and aphasia Now, my question is this, is there enough overlap that this can be called migraine and not epilepsy, because I'm seeing my neurologist this Wednesday and I'm going to put the question to him and if he brushes it off like he often does I wan't to know whether to push for an EEG or not. Regular Visitor Posts: 428 05/04/08 9:30 PM	Jim, My neuro-ophthalmologist is working under the assumption that this is seizure/epilepsy related in some way. She said that anytime the brain is not interpreting what it should in the manner it should, then it is not wired and firing correctly...she postulates that this is a variant, however distant, to epilepsy. The only term I have found explaining this is "sensory epilepsy" - when use or overuse of any of the senses causes the brain to go into a type of seizure mode. There is also an autonomic epilepsy that causes unexplained heart palpitations that several members of this forum experience...maybe it is all related somehow?
